The Comprehensive Eye Section provides general ophthalmologic care in an academic, private practice setting with in-house access to world renowned subspecialty expertise both within ophthalmology and other areas of medicine. In addition to caring for patients with general ophthalmic problems and routine eye exams, the Section provides executive physicals as well as coverage for walk-ins and Yale-New Haven Hospital Emergency Room ophthalmic emergencies. The Comprehensive Eye Section also handles inpatient consultations for Yale-New Haven Hospital. The team of the Yale Low Vision Program also works closely with the Comprehensive Eye Section.

Your Appointment: What to Bring with You

The Yale Eye Center physicians ask that you bring the following items with you for your appointment:

• Your current eyeglasses and/or contact lenses
  
  
• List of all current medications (include the name of the medication and the dosage)
  
  
• Medical records that relate to your current condition. This should include X-rays, CT scans and any additional test results or copies of medical records
  
  
• Your current health insurance identification card and any referral/authorization forms that may be required by your insurance company.
